De term persisterende trofoblast is vervangen door de term Gestational Trophoblastic Neoplasia (GTN).
Er is een GTN wanneer bij hCG follow-up na evacuatie van een molazwangerschap sprake is van:
- een plateau van het hCG in 4 opeenvolgende wekelijkse metingen gedurende 3 weken,
- óf als er een stijging van het hCG is in 3 metingen gedurende 2 opeenvolgende weken,
- bovendien dient 1 van de waarden buiten de P95 van de regressiecurve te vallen.
Zeldzame GTN, zoals placental site trophoblastic tumour (PSTT) vergen een individueel beleid:
- Primaire behandeling bestaat uit een hysterectomie,
- Uterus sparende behandeling middels bijv. neo-adj chemotherapie, moet als experimenteel worden beschouwd.

Bij methotrexaatresistentie is actinomycine D een goed alternatief, indien er nog steeds sprake is van een laagrisico GTN.

2e lijn: Dactinomycine D. 0.5 mg bolus d1-5 à 2 weken (2-4)
- Controle beta-HCG voorafgaand aan iedere nieuwe kuur. Hoort logaritmisch te dalen. Na normaliseren van beta-HCG nog 2 consolidatie cycli,
- Follow-up met maandelijks HCG-controles gedurende 1 jaar,
- Na normalisatie van het HCG mogen patiënten gedurende 1 jaar niet zwanger worden. Gedurende dat jaar en direct na normalisatie van het HCG moeten zij anticonceptie gebruiken.

Etoposide + Dactinomycine + Methotrexaat + Vincristine + Cyclofosfamide, 2-wekelijks (EMACO) (3)
- Etoposide, 100 mg/m2, NaCl 0.9% ½ uur, d1-2,
- Methotrexaat, 100 mg/m2, bolus, d1,
- Methotrexaat, 200 mg/m2, NaCl 0.9%, 12 uur, d1,
- Dactinomycine, 0.5 mg, bolus, d1-2,
- Folinezuur, 15 mg/m2, 2dd po, d2-3,
- Cyclofosfamide, 600 mg/m2, NaCl 0.9% ½ uur, d8,
- Vincristine, 1.0 mg/m2 (max. 2 mg) bolus, d8.
- Controle beta-HCG voorafgaand aan iedere nieuwe kuur. Hoort logaritmisch te dalen. Na normaliseren van beta-HCG nog 3 consolidatie cycli.
NB. Indien beta-HCG-liquor-serum ratio> 1/60, CAVE hersenmetastasen. Bij hersenmetastasen tevens radiotherapie van de schedel.
NB. Alkaliseren urine: gedurende 72 uur natriumbicarbonaat 4 dd 1000 mg.

Etoposide + Cisplatine + Methotrexaat + Dactinomycine + Folinezuur 2-wekelijks (EP-EMA) (4)
- Etoposide, 150mg/m2 iv in 1000 ml NaCl 0,9 % in 1 uur, d 1.
- Cisplatine, 75mg/m2 iv in 1000 ml NaCl 0,9% in 3 uur, d 1.
- Etoposide, 100 mg/m2 iv in 1000 ml NaCl 0,9 % in 1 uur, d 8.
- Methotrexaat, 300 mg/m2 iv in 1000 ml NaCl 0,9% in 12 uur, d 8.
- Dactinomycine, 0,5 mg iv in 250 ml NaCl 0,9% in 30 minuten, d 8.
- Cumulatief maximum etoposide is 2000mg/m2 ivm secundaire hematologische maligniteiten.
- Controle beta-HCG voorafgaand aan iedere nieuwe kuur. Hoort logaritmisch te dalen. Na normaliseren van beta-HCG nog 3 consolidatie cycli.
- Follow-up met maandelijks HCG-controles gedurende 2 jaar.
- Na normalisatie van het HCG mogen patiënten gedurende 2 jaar niet zwanger worden. Gedurende die 2 jaar en direct na normalisatie van het HCG moeten zij anticonceptie gebruiken.
Hysterectomie als behandeling van patiënten met GTN is te overwegen bij:
- Falen van chemotherapeutische behandeling bij een aanwezige massa in de uterus,
- Behandeling van levensbedreigend bloedverlies,
- Vrouwen zonder kinderwens bij voorkeur ouder dan 40 jaar.
Na een hysterectomie is het nog steeds essentieel om het serum HCG te blijven vervolgen.
