Oesofagus­carcinoom

Lokale behandeling

• Behandeling resectabel oesofaguscarcinoom

  ‘Locally advanced’ oesofaguscarcinoom of oesofagogastric junction tumor (cT1N1M0 of T2-3N0-1M0).

  Neoadjuvante chemoradiotherapie met paclitaxel en carboplatin: “CROSS” (van Hagen 2012).

  Behandelschema:
  Paclitaxel 50 mg/m2 in 250 ml NaCl 0,9% in 1 uur en Carboplatin AUC = 2 in 250 ml gluc 5% in 1 uur. Intraveneuze infusie op dag 1, 8, 15, 22, en 29. 

  Radiotherapeutische behandeling:
  Totaal dosis van 41.4 Gy in 23 fracties van 1.8 Gy en 5 fracties per week, te starten op de eerste dag van de chemotherapie. 

• Behandeling irresectabel oesofaguscarcinoom

   ‘Definitieve chemoradiatie’ (oncoline).

  Bij irresectabele tumoren (locatie: cervicaal, histologie: plaveiselcelcarcinoom) en bij niet- operabele patient (KI<80, veel comorbiditeit): carboplatin/taxol vlgs cross maar dan gedurende 6 weken ipv 5.

  Behandelschema

  Paclitaxel 50 mg/m² in 250 ml NaCl 0,9% in 1 uur en Carboplatin AUC = 2  in 250 ml  gluc 5% in 1 uur. Intraveneuze infusie op dag 1, 8, 15, 22, 29 en 36.

  Radiotherapeutisch behandeling

  Totaal dosis van 50.4 Gy in 28 fracties van 1.8 Gy en 5 fracties per week, te starten op de eerste dag van de chemotherapie.

Gemetastaseerd oesofagus­carcinoom

• Eerstelijnsbehandeling

  Lokaal recidief en/of metastasen

  • CPS score bepalen
  • Indien adenocarcinoom: Her2 bepalen.

  HER-2 positief

  Behandelschema 1e lijns: Capecitabine, oxaliplatin met Trastuzumab ‘TOGA’ (Bang 2010).

  Capecitabine + Oxaliplatin (3-wekelijks)

  • capecitabine: 1000 mg/m2, 2 maal daags dag 1-14,
  • oxaliplatin: 130 mg/m2, dag 1, 
  • Trastuzumab 1^e dosering 8 mg/kg dag 1. Volgende doseringen 6 mg/kg (dag 1).

  Na 6 cycli Trastuzumab monotherapie continueren.

  Bij progressie Capox hervatten, geen evidence voor continueren trastuzumab na progressie

  HER-2 negatief of plaveiselcelcarcinoom en CPS<5

  Capecitabine + Oxaliplatin (3-wekelijks) (Cunningham 2008, Dijksterhuis 2019)

  Indien in goede conditie (KI>90):

  • Oxaliplatin,130 mg/m2,
  • Capecitabine, 1000 mg/m2, 2 dd gedurende dag 1-14.

  Behandeling gedurende 6-8 cycli.

  Alternatief: Capecitabine monotherapie.

  Capecitabine 1250mg/m² 2 dd gedurende dag 1-14.

  Bij passagestoornissen:

  Overweeg gemcitabine / cisplatin (3-wekelijks) (van Zweeden 2018):

  • gemcitabine 1250 mg/m2 (i.v. dag 1, 8), 
  • cisplatin 80 mg/m2 (i.v. day 1).

  HER-2 negatief en CPS>5

  Capecitabine + Oxaliplatin + nivolumab (3-wekelijks) (Janjigian, Lancet 2021)

  Indien in goede conditie (KI>90):

  • Oxaliplatin,130 mg/m2 dag 1 IV
  • Nivolumab 360 mg dag 1 IV
  • Capecitabine, 1000 mg/m2, 2 dd gedurende dag 1-14 per os

• Tweedelijnsbehandeling

  Gastro oesofageale overgangstumor / distaal) adenocarcinoom

  2^e lijns behandelschema: Ramucirumab paclitaxel (4 wekelijks) “RAINBOW" (Wilke 2014):

  • ramucirumab 8 mg/kg IV dag 1 en 15,
  • paclitaxel 80 mg/m(2) IV dag 1, 8, en 15.

  Continueren tot progressie of onacceptabele toxiciteit. 

  Plaveiselcelcarcinoom

  Geen standaard tweedelijns behandeing op basis van CieBom advies (ATRACTION-3; Kato 2019).

• Derdelijnsbehandeling

  Gastro-oesofageale overgangstumor / distaal adenocarcinoom

  Trifluridine/Tipiracil (Shitara 2018):

  Trifluridine/Tipiracil 35 mg /m2 dag 1-5 en 8-12 van 28 daagse cyclus.

  Nadien

  MSI bepalen voor evt studiebehandeling.
